Project Description
The target populations for this project are the four public housing/senior
sites in Marion County as well as the surrounding community. While the Health
and Hospital Corporation of Marion County is the grantee of record, services
are provided by Citizens Health Corporation via a co-applicant agreement.
Comprehensive primary health care services are delivered at the Barton Annex location. Transportation to and from the health center is provided to residents living in the other three housing/senior sites. The medical team consists of a physician, a registered nurse, and a medical assistant, with a social worker providing case management services. All public housing residents have access to full dental services through the dental department located at the College Avenue location.
