Project Description
Curtis V. Cooper Primary Health Care, Inc. operates a health center located in
downtown Savannah, which is within close proximity to several public housing
communities. The emphasis of the Cooper Primary Health Care PHPC project is
increasing health care access through health promotion and disease prevention
activities. This model utilizes outreach staff to educate public housing
residents and encourage regular visits to the health center.
Services
Primary Health Care for adults, preventive care, chronic and acute care,
prenatal care, pediatric and well child care, health screening and care for
acute illness, supportive services, enabling services, transportation, nursing
case management, outreach, laboratory services (CLIA waived laboratory onsite -
additional laboratory services available at main clinic site), pharmacy access,
and 24-hour coverage for emergencies and hospitalization, if needed.

Project Description
The West End Medical Center Public Housing Primary Care Program has provided
on-site primary care services to public housing residents for the past twelve
years. A partnership with the Atlanta and Cobb County Housing Authorities has
afforded the greatest accessibility for residents in five sites.
