Project Description
QNHOP's PHPC program provides holistic health care and mental health care to the residents of Quigg Newton Homes and the Sunnyside neighborhood in northwest Denver. The program provides a complete range of Family Practice care including pediatric, adolescent, adult, geriatric, and obstetric care. Mental health care is provided on site and access to substance abuse, dental, and eye care are provided by referrals.

Services
Door-to-door outreach is provided to the Quigg Newton/Sunnyside residents by a full time Outreach Worker. An outreach worker or case manager conducts home-visits and completes initial assessments, offering access to available services or, if needed, the provision of limited in-home care. Transportation services are offered to and from the health center, the hospital, and to other service providers for patients. Dental and Optical services are offered to Quigg Newton/Sunnyside area residents who cannot afford to pay. The Senior Watch Program provides services to the seniors in the Quigg Newton/Sunnyside area.

Nutritional counseling and smoking cessation classes are provided through Denver Health on- or off-site. Domestic violence support groups are provided on-site for community residents.

BPHC Collaborative Participation
The PHPC program is involved in the Diabetes Collaborative and has recently begun assessments as a part of the Pediatric Asthma Collaborative.
