Project Description
The PHPC Health Centers of Birmingham Health Care (BHC) provide urgent, primary preventative health care to the residents of the public housing and surrounding communities. Ancillary and supportive services are provided at BHC or through referrals to other service providers in the community.

Services
Urgent, Primary, and Preventative Medical Care. These services include laboratory services, diagnostic testing/screening, family planning, HIV screening, testing, counseling, follow-up and referral, immunizations, hearing and vision screenings, nutrition counseling, TB services, health education, follow-up, and referral, and 24/7 on-call coverage

Linkages/Partnerships
BHC has established multiple collaborations in the community including collaborations with the County Indigent Hospital, local Health Department, homeless shelters, mental health service providers, and substance abuse treatment facilities. Further, BHC collaborates with colleges and universities in the community, such as the University of Alabama at Birmingham to provide adult and pediatric eye care.

Project Description
The barriers which have traditionally prevented public housing residents from receiving primary health care have been reduced, due to the establishment of health centers on-site in public housing communities. Health centers have been opened in two of the three public housing developments and are fully staffed with family medicine providers. A full-time social worker coordinates the delivery of educational and health services.

Services
Pediatric, adult, and geriatric medicine, transportation, dental screenings, medical home visits for the elderly; educational seminars, games, and exercise classes, tutoring and reading classes for children, parenting classes, referral to medical specialists
